Register the student-athlete on DragonFly
Select the link below to open the DragonFly registration form.
DragonFly Registration -
Print the NCHSAA Student-Athlete Preparticipation Physical Evaluation Form.
- Parent/Guardian and student-athlete must complete and sign the COVID questionnaire and the History Form (questions 1-32).
- Health care professional must complete the Physical Examination Form and Medical Eligibility Form.

Free physicals are available from Blue Ridge Health. To receive a free physical, parents/guardians must complete the Blue Ridge Health Registration/Permission Form in addition to the COVID questionnaire and History Form on the NCHSAA Physical Evaluation Form.
